How PlacementPoint Works
A referring professional;s uch as a Care Manager, DSS Social Worker, Hospital Discharge Staff, or Court Counselor submits a referral for a youth needing out-of-home placement. The referral process typically takes 15–20 minutes to complete.
Once submitted, Rapid Resource for Families (RRFF) reviews the referral for completeness and then routes it to the appropriate provider network based on the youth’s health plan and catchment area.
Provider Agencies review each referral and respond with a disposition:
- Yes – A match is possible, and the agency can accept the youth.
- Maybe – The referral is under review and needs additional time for clinical staffing or internal matching.
- No – The agency cannot accept, and provides a clear clinical or logistical reason.
PlacementPoint also facilitates direct communication between providers and referring professionals, once mutual interest and need are expressed. This ensures clarity, strengthens coordination, and ultimately supports faster, better placement decisions.
